PATIENT INFORMATION
Adult New Patient Visit
Your first visit is all about you: your comfort, your happiness, and your health. During your first visit, we will review your dental and medical history forms. Our hygienists will then take the necessary radiographs (x-rays) and gather information about the health of your mouth. Dr. Snyder will then meet with you to discuss your own individual treatment plan, and answer any questions you may have.
The Ardente Membership
About 1/3rd of adults in the United States do not have dental insurance. To help fill that gap, we have created a Membership plan. The Ardente Membership allows you to make low monthly payments for your preventative care and also gives you discounts on other treatment in the office.
Insurance
As a courtesy to our patients, we accept most dental insurance plans and will gladly file a claim. Co-payments and deductibles (the portion the insurance does not cover) are due at the time services are rendered. In order to process claims properly, patients must provide us their correct and appropriate insurance information and inform us of any changes regarding their dental insurance at the time of service. Please keep in mind, most dental insurances do not cover cosmetic procedures.

At ArDente Dental we are dedicated to making your services as easy and affordable as possible. We feel that it is very important for our patients to be educated and informed about their dental care needs.
Financial planning for your dental health can be a substantial purchase. We would like you to be well informed about the cost and payment options available to you. Our qualified staff is available to help you choose the payment option that best fits your needs.
Forms of payment we accept include Cash, Check, Credit Card (Visa, Mastercard, Discover), Debit Card, Flex Spending, along with Care Credit.
